Bring the following items:

Insurance Card
Driver’s License
Rx (Script) from your referring Doctor
- Please check with our front desk. Your doctor may have already faxed it to our office.
Dress comfortably with loose-fitting clothes.

List of Medications
- Include any vitamins and/or herbal supplements, with dosage and frequency.
- Note: If your insurance requires a PCP referral, please let our Front Desk know prior to your initial appointment.
Arrive 15 minutes before your scheduled time.
